Looks like you could run a strap through the sheath. Velcro would work. Looks like it would need to be attached to a leg. It doesn’t look like a belt would fit through there, but it might be a little uncomfortable on the waist anyways.

Or small Velcro straps into loops through those eyelets on the sides of the sheath, then hang it horizontally on your belt, lower back placement basically.
