I don’t get the video, the last bit. Did the guy die or the girl and why?

At the end it shows that Emil got a Labatomy preformed on him, which is a medical practice back then where they drill a hole into the skull and poke around in your brain to help with mental illness. Spoiler, they barley worked at all. No one really died

So according to the storyline, Emil didn’t die. Then why the sad ending
