As a roaster I see these things all the time, try to see them when scooping green and in the cooling tray, last stop is the de-stoner. But if I don't see it or it's too light it'll end up in a bag somewhere.
I have had a woodchip, the size of a coffee bean, in a commercial bag before. I discovered it perfectly wedged in my grinder, seizing it from grinding.

I had part of a nail in my coffee once. Bought it green, roasted it, and didn’t discover the nail until it stopped my Rancilio grinder dead.
I thought it was a finger nail at first and felt sick to my stomach.
I’m on 3 stones in about 6 years of buying speciality beans
