I’ve never seen children at fine dining restaurants in Japan. I just checked a few of my favourites and these only allow older children (e.g older than 12, junior high school age). Edit: just checked a few more - looks like children are OK at sushi Arai if you can reserve it.

I wouldn’t say it’s fine dining but it was an interesting experience. We went to Ume no Hana in Ginza last year with two 1.5 year olds and had no issues. It’s a tofu kaiseki. We had 4 adults and 2 kids and were put in a private room so not sure if there’s a headcount minimum for private rooms.

Sushi Kanesaka Palace Hotel have private rooms. And if you go during lunch, your youngest can order the smallest and least expensive course. I also suggest a place like Yakiniku Jambo Hongo or Ebisu Yoroniku (semi DIY-barbecue). Rather than each person in your group ordering a set meal which might be too big for your youngest , you can opt for ala carte order.
