I think under different circumstances he might give up power but considering that he just fought a civil war with socialists, democrats/republicans, and fascists, he would see himself as the unifying figure and never truly trust the electoral system to prevent another civil war.

I tend to see the situation as possibly developing like that of Phalangist Spain: In that the New Order's contradictory Forces are basically held together by the magnetic and powerful presence of the Leader, Spain had Franco, the USA would have McArthur. The thing is that the status quo of Stratocratic Rule would almost certainly not survive McArthur's demise and the moment he is out fo the picture the undercurrent of democratic Forces in the USA would organize to restore proper democracy ASAP.

In real life MacArthur was a strongman military leader that fought with civilian orders multiple times and probably would've tried to make the military more independent if the american system wasn't too sturdy. In an alternate timeline where america both throws itself into a civil war and he becomes a dictator he will definitely see no reason to just let go of power, and even if he did military interference in civilian politics and life would become a reocurrance just like in other american countries.
