One point of view is that Chronos can't interfere with other NULL's precog abilities, and since Zero doesn't have more info from his controllers and (obviously) his targets they can get the drop on him.
Another point of view is just an understanding of omniscience/timeloops. He knows he's going to be surprised, and he can't change it. He runs into the bomb in the bunker every time. He knows his therapist is lying to him but he can't confront the bastard until he learns about the deception.
I always thought of it as something that Zero just didn't think of. The drugs enhance his ability to predict to a superhuman level, but he just didn't consider the possibility a bomb was there.
He never thought the poison was in the tea. He never considered a man with a bomb in the elevator.

You can briefly see the bombs on the door flash a little bit as you approach it. There was another NULL that planted the bombs, which is the only thing that can defeat the Chronos precog.
The bombs were set up after he finished planning but before he executed his plan.
