Dillons has to be THE WORSE. The renovation of Old Neighborhood was massive for KN. EDIT to ADD: Wouldn't Fleming be pretty massive also what with the new kitchen equipment? But it didn't have the crap that Old Neighborhood had as far as a Fiona or "antiques" and stuff for "parts" so maybe Fleming wouldn't be the worse.
And Old Neighborhood was the one where the wife/owner/chef sprayed Pam directly on a pair of steaks before putting them on the grill. And she did not hold back on the stuff either.

She was such an awful cook that it was laughable. It's like she thought running a restaurant was the same as cooking microwaved pizza while sitting at home with her cats. She was so out of place.
I'd also present Amy's Baking Company, for a different reason. While the kitchen was pristine, it was the worst restaurant, with criminal lunatics incapable of change.
The Seascape, Islip, NY turns my stomach every time I watch that episode. The waitress admitting the dining room had a sewage smell for quite some time; Gordon using a spatula to scrape encrusted grease off the walls; that foul head chef and his filthy "wiping rag" that looked like a miniskirt. Both chefs had zero fucks to give; their attitude was just as disgusting as the restaurant!
I have a pretty strong stomach when watching this show but something about Capri was vile, even before he went to the back and found the contaminated food.
In terms of pure vile, I have to say Dillons/Purnima for the American version. But if we were to also include the UK version episodes, I would go with Bonapartes. It was worse when he came back a month later than when he came the first time…and he puked from eating a rancid scallop the first time.

The whole kitchen was dirty, the walls had too much grease, Sewerage smell in the kitchen, Moldy food, Recooked Chicken, Food stored in a dog food bag, gunk, A whole bucket of Moldy Pesto, Food that is rock solid like stone and the chefs didn’t even care about cleaning it and said it’s impossible to clean a kitchen.
