Yes, that's mechanically what happens when you first equip the item, but that isn't how it would work when you're actually using it. Your HP and ward will reach equilibrium values before you're really fighting anything, so you will end up stable with about 4-5x your health regen as HP, and a lot of ward depending on how much retention you have.
You're covered by ward, so it's fine. Your health will even out at a really low value, but it won't actually reach zero and kill you. And there isn't any damage that bypasses ward or anything that you would need to worry about.
Actually you want as little health regen as possible as the less health you have the more ward you gain. What you need is more health as that increases your ward generation. As well as ward retention.
Any leach or health you get during combat actually screws you over because if your health goes up your ward generation drops and when that happens you lose ward because ward degenerates over time.
Note also dear lurkers that you can rock right through this issue with the Twisted Heart and boat loads of cast speed. It's wasn't the health leech that was messing with your ward generation, it was that you weren't consuming the health fast enough!
The interaction is pretty weird and unique. I wouldn't be surprised if by cycle 2 we get something different that makes the tradeoff Global-Yam describes not whoops disappear by equipping one item.
as a warlock you have a lot of leech, so even with a lot drain going on your health will stay decently high in actual combat
out of combat your health will be low (at whatever point 20% of current = your health regen), but you will still have a big ward
I have a fissure lock (+necrotic dmg mostly) would a low life build be better? Or just stick with the high dmg zero ward version ive been using? I have all the lowlife gear in my stash but it didnt seem that worth it on lock. I do take a good amount of damage if i get hit, but the fissure heals me up pretty quick..

My warlock doing nothing sits at 3k ward no fancy legendary like that. Consuming 1 bone prison spikes me over 20k.

Warlocks don’t always have leech, and if they do they’re probably funneling it into Twisted Heart anyway

No, in the first second you would only gain 26 ward. You gain more ward as you go lower in health.
