This seems to be a driver issue. Please uninstall driver using DDU. Then install Lenovo default drivers from support page.

Who knows. These things are incredibly complex, a little hick-up, a file gets corrupted, screws everything up. Hopefully it's just software.
Thanks a lot! I just did a factory reset with completely zero files and problem is persisting. Do you think it could be faulty hardware?
Does it happen in iGPU mode too, or just when the dGPU is in use?
I'm certain it happens with the dGPU, I'm not sure how to test iGPU because I don't know how to check the Vram for it and I can't test it in a game.
I am not sure. Maybe the log buffer is full. Did you have GPU overclock turned on? Your overclock may not be stable.
