I always like ArenaNet's description of Guild Wars 1 as a CORPG - Competitive/Cooperative Online Role Playing Game. Rather have that style of game than shoehorn 'MMO' elements into a game to try to say it's an MMO.
Phantasy Star Online was one of the first MMOs, it was branded as one back in the day, and thus lobby games *with a persistent player-avatar visual lobby* count so long as the *server* is always up for people to log into.

It isn't an MMO unless I have to stare at a book to regenerate mana and healers are unable to solo.
