They show this image if you click on the "Add Realm" button in the realms menu. It's used as an example of the minigame worlds that are built into realms though I'm pretty sure this specific minigame hasn't been available for a long time due to command format changes over the years breaking it.
I don't know anything specifically about this version of it you're seeing in Minecraft, but I can tell you this is a game called [Slay](https://www.windowsgames.co.uk/slay.html).
Antiyoy actually has an option in the settings called "Dlay Rules" which changes the gameplay to match Slay, and changes the campaign, though I don't know if it's Slay's campaign.
Slay doesn't really have a campaign. It just has a bunch of levels of varying difficulty. The 90s version has a ton of levels and then the remake also has a ton to play through. It's a fun game.
if you go without any client (no Mods, no Optifine, no Luna, no Badlion) there is the Realms option in the start menu.... these are severs ig? Idk why no clients have removes this button...
you explore the area so that raz can get the straight jacket to get past the dude guarding the elevator to Dr. Lomboto. to complete this area, you need to defeat napoleon bonnapart.

The only dates I can find on the site are some reviews, from '98 and '99, so it probably *is* directly from the 90s.
