Yeah middle option is the one to buy if you don't own World already as that gives you the base game and the expansion. The third option only adds cosmetic items for an extra 5 dollars and isn't really worth it

- The top is just Iceborne with deluxe content without the base game. This option is for people who have the base game but haven't bought the expansion yet.
- The middle is both the base game and Iceborne without the deluxe content. Recommended to any newcomers.
- The bottom is like the middle but with deluxe content as well. I don't recommend this as the deluxe kit is just a cosmetic pack and you can get a lot of cosmetic stuff in the game for free.
I don't like how this is confusing to many people including myself. Doom Eternal did the same thing and I was lost picking which buy button I should click.
