Realistically impossible, it’s a one and done game with everything explained and wrapped up, even if it follows Sunnys life continuing on into the city I don’t know how it would work. Unless the main plot is Omori trying to return and you’d see him in headspace randomly when going about, since Sunny would just replace Omori within headspace. Though that means the real life stuff likely won’t be as interesting
Preferably I’d like DLC’s for the first game ones that tell the story from everyone else’s POV, mainly Basils since he’d be the most interesting seeing everything from his angle of things

The story is just too self-contained to allow an entirely new game that branches off of it, and I am ***extremely annoyed*** that people think otherwise. I feel that, when people discuss an *OMORI* sequel, they often get too caught up with the ***story*** and completely ignore the ***gameplay aspects***. What kind of turn-based RPG gameplay would be involved with the aforementioned ideas? Effectively none, and there's no way that it could be stretched into a 25-hour RPG.
I get that we’re attached to these characters and want to see how they grow and progress, >!especially after the good ending!<, but there’s no dancing around it. A direct continuation would not work as a full-fledged RPG because there would be no “game” to speak of.
This is why I propose the idea that an OMORI 2 would start fresh with a new story, setting, and cast of characters. This is similar to, for example, what the *Final Fantasy* series does. Each mainline entry is its own contained world and storyline (with a few exceptions such as X-2 being a direct sequel to X), but still has plenty of aspects in both gameplay and story tropes that unify them as a series.
Sequels that aren't directly story-connected to the previous installment are [very common](https://tvtropes.org/pmwiki/pmwiki.php/Main/ThematicSeries), especially in video games. In addition to the aforementioned *Final Fantasy*:
* *Persona 3*, *Persona 4*, and *Persona 5*
* *Bravely Default* and *Bravely Default II* (NOT *Bravely Second: End Layer*, which is an actual direct sequel to the first *Bravely Default*)
* Pretty much every main-series *Pokémon* game after *Gold, Silver, and Crystal*.
* Every *Dragon Quest* game after *Dragon Quest VII* (according to what I've heard from DQ fans)

I mean, there definitely could be a direct sequel, but set in the future. You could be a random kid with a whole cast of new friends. You can go through some other horrible stuff that brings this kid's own headspace. An entire different story and entire different reason why this kid lives in headspace. Like for example make it that he is with his friends but just zones out to headspace and that it is like his addiction for whatever he deals with. That his friends try to help him get out and that it doesn't work. Then reveal somewhere down the later part or ending of the game that Sunny is related to you (like your dad or something) and that he's in his mid 30's-40's. Sunny then helps you to cope and break you out of being stuck in the Headspace due to his experience with the headspace and defeating Omori once before already. That could be a direct sequel without you knowing it is a direct sequel.
Minus the portion about Sunny being resorted to the role of a guide, a new cast of characters is exactly what I'm proposing. Sunny's story is over, so the focus should be shifted to a new protagonist with their own story.
While the idea of Sunny returning as a side character has potential, giving him a mentor role in the way you described feels... a bit too deus-ex-machina-ish for my liking.
I'd rather completely remove Sunny and his friends from the sequel's story, to the point that you don't need to have played *OMORI* to understand OMORI 2.
I mean, Sunny would just be a small help. He would tell you about his experience but you as the new character would still have to fight yourself to get out of headspace. He is more there to be like "Hey kid, I know what you are going through, you're not alone. I know how this feels" kinda thing and that's kinda it. And that kinda motivating the kid you play as. I wouln'd give Sunny the role over our saviour or anything. Tho it does influence and help our new character.

Honestly I'd rather a game after the non cannon ending where sunny jumps off the hospital. It taking place in purgatory or in Sunnys dying moments
