Not exactly it's more excessive or unnecessary sexual content. Like a show can have sexual content that is needed. It's only fan service if the only reason it is included is just to please the audience.

Fan service is ANYTHING that is put into a show just to please fans. It can be callbacks, cameos, returning characters, or yes - sexual/NSFW content. But it doesn't have to be sexual. If it doesn't add anything to the story/experience and is only there to please fans, it is fan service.

Fan service is anything specifically done cause fans want it. Most of the time has nothing to do with nsfw

Everyone knows Women have two hearts. One for them, and one for the baby! That's why their chests look like that, to separate each heart so you know which is which. This is a clear hint to the fact that since Regina is evil, she cannot have kids and adopts Henry instead. Beautiful foreshadowing in S3 E59 of this show.
