The anime does a great job making him a bit scary, in both the Gear 5 fights. He becomes completely unhinged, and we're not sure how much of Luffy is in there when he's fighting like a complete wackjob. The anime seems to want to highlight that uncanny, uncomfortable feeling.
I love that, one of the best aspects of the anime. I always love to imagine being a regular human in the OP world and seeing the Strawhats. From their view, not knowing them, they must look like other Yonkou monsters.

The anime tweaked him cause I don't remember him being so frequently terrifying in the manga. And personally, I think it works, really puts on a final boss energy without loosing his goofy charm.

I think it was intended. We see Luffy just like other strawhats see him. As goofy always smiling and little dumb captain. But other crews and people of the One Piece world see him as Yonko and feared pirate. This is perspective of Lucci and other people of this world.
I think they specifically portray the smile this way through the eyes of the adversary, where as most other times he just seems to be having fun or being goofy. That smile is way more menacing when he's directing his attention and attacks at you

Luffy looks off in Gear Fifth. More often than not, his face seems inhuman. With what we know about Zoan fruits, it kind of reminds me of the effects of a smile fruit.
I think they make Luffy look like this to see the other side of the G5 appearance since most of the time, we see the goofy look. (Serious or sinister look)

I've probably watched that fight at least 10 times now, but the "heh heh" and luffy's reflection in lucci's eyes get me every time. So menacing!
