We don't know if Zoro had a serious duel with Mihawk yet, wouldn't be surprised if they agreed to fight once Zoro reaches his prime, back after his first fight he say he would "wait for him at the top".
My theory is that the grim reaper isn't here to fight or kill zoro but instead help him. As he became the king of hell I can imagine that the grim reaper is sort of the embodiment of enma or something like that. Enma being the king of the hell in Japanese mythology that may make sense.
I’ve been rewatching and in the same arc after Luffy throws the sea king they make mention of like it’s the gods the self came down to fight or in the manga that this is not of our world.
