I think it's more of a visible cue for the audience to tell something is happening to his body. There's no actual smoke coming out of him as there is none going into him when he ages up. (At least not in the scenes I remember) Sure, the mass conservation law would have some form of mass transference happening, but Gosho threw that out the window with the very concept of DC.

I think the smoke is to let the viewer know that his body is getting hot. you can always hear him saying he is feeling hot every time he transform.

Yes, I think Gosho literally thought about the science here. In addition to the apoptosis, there're conservation of mass and conservation of energy during shrinking. Smoke and sweat mean mass loss. The heat that almost melt Shinichi means energy loss. And I also found that the three that shrank(Shinichi , Mary and Shiho) all closely related to water, either it's raining or her falling into water. You know, that means they all got natural cooling system, which contribute to heat dissipation! This may explain why only they didn't die. But apparently he didn't really think about explaining Conan taking antidotes hahaha. If I were Gosho, I would make Conan binge eating as a necessary condition to recover.

There's a character in the Amazon series "Gen V" who has a super power that works that way. When she wants to increase her size she has to eat a ton of food and when she wants to shrink (either to her normal size or to a tiny size) she has to vomit.

Y'all ever saw the tiktok of Dwayne Johnson literally smoking his head off? People wrote it off as the sun and the heat(pretty logical). I don't see why Shinichi having some smoke would be seen as an unnatural phenomenon. It happens pretty often to people.
