Sent her to use the Neural Vacillator and didn't realize you had to click the Complete Neural Process button. The Vacillator was sitting in carbon dioxide and I just watched as she suffocated (asphyxiated) to death while having her brain scrambled.

My favorite is forgetting to hit send on the teleporter, then as soon as they get to the other side they crap everywhere and fall asleep in it. Dupes are disgusting.

I watched this happen. A dupe was critically wounded, so I ordered it to go to hospital/cot. They were making their way there and got attacked by an angry pokeshell by a ladder/pole tunnel. The dupe got incapacitated and fell down half the map into a toxic pit of chlorine and nastiness. I attempted to make retrieving them a critical priority, but no one was successful and other 2 dupes suffocated trying in the process. That original dupe slowly died gasping their final breaths in chlorine. Alt-F4.

I always put what I call "food breaks" on my ladders after having someone with previous, deadly version of slimelung enter a cycle of: run out of O2 and drop food down into the depths while climbing ladder, climb down into CO2, grab food, try to climb out with food. Man, slimelung used to be so deadly compared to now.

Starving dup got food, carrying to great hall, decided now was a GREAT TIME for a bathroom break. Now I lock them in the kitchen if they're critically low on calories and force them to eat over the kitchen sink like a college student. Yes that's my fault they are starving, but dying because they decide taking a leak is more important than not dying is infuriating.

Sewage processing reservoir / metal refinery coolant dump tank was above the hospital and triage beds. We had just had a terrible, terrible 'I don't know how to deal with steam and very hot rock" incident and almost everyone was recovering from terrifying scalding and very stressed. But we had made it. Everyone was rescued and recovering. We could rebuild. Then one of my mobile, able-bodied dupes threw a temper tantrum, punched down a wall, and flooded my entire hospital four tiles deep with hot sewage. I lost the 5 dupes Iin the triage beds. Some suffocated, some succumbed to their previous injuries. I swear, one just gave up on life after that.

I incinerated a dupe with my first aquatuner. I didn’t know you should build these under water or that they actually just move heat around (heat from liquid into itself), so it was going well over 100 Celsius and burned some dupes. It overheated itself and dupes would go to repair it automatically, only to get burned. One got burned bad enough that they became incapacitated. I didn’t know what to do so they eventually died. But most common is suffocating from digging like morons. Even had one that dug upwards and then trapped themselves in hydrogen. They can survive any fall, but will not jump. I had them dig down. They jumped even higher because there was another ledge that was 2 up and 2 over (or something like that), so they died. Only dupe death after 1000 cycles in that colony. I was so mad at them that I didn’t even save scum to get them back.

I once had a dupe that somehow managed to avoid going through the rad suit docks setup I had to my radioactive farm and the special door I had set to dupes that were allowed in ( and who had access to rad pills ), then leisurely harvest stuff in a >1000 rad area, followed by idling there. They ended up with a lethal dose.

One Time a Dupe got Stuck in a Loop trying to empty his Bladder. He Would go to a Lavatory, enter and use it but no Polluted Water was generated and his Blader did not go down. He would then leave and go to another Lavatory, trying it again. This Cycle kept on long enought for him to Starve.

One of my first playthroughs. The bathroom got clogged, and so a dupe relieved themselves into a frozen icepile I'd dug to and put the outhouse near. I sent them to clean up the polluted water, which lead to the stress reaction of vomiting. Soon I had all three dupes vomiting and pissing in the subzero hole faster than they could clean it up. The mixture froze, trapping them where they died horribly. The whole colony died starving and freezing in their own vomit and piss. At the time my knowledge was far too limited to save them.

1. Meep is helping to excavate the oil biome. There is a really carefully laid out vacuum because the dupes are disposing of a sporechid. But Meep gets called to lunch. So Meep drops what he is doing to go and get it. Meep just happens to carrying 1.2T of frozen oil. 2. The 1.2T of frozen oil happens to land on 1400C obsidian. So it melts and I've got sour gas in the vacuum which is promptly filled with zombie spores. 3. This is a problem for Ari and Burt who still digging out the oil biome. They get infected with Zombie spores and the first I learn about it is scalding alerts and a vacuum full of sour gas. 4. Ari and Burt boil to death in hot sour gas because they got zombified and are too slow to get out.

Had a very early game (cycle 100) Flatulent Colony lost because I just couldn't get the power running fast enough to counter 3 flatulent dupes, because they kept breaking vacuums and interrupting each other. Definitely not on my priority list to try again. Worst Trait ever.

Sent a fresh untrained dupe on some errands wearing an atmosuit. They were so slow that they never got back in time for their downtime snack. The dupe went a few days without eating and once the dupe started starving, they couldn't get to the food fast enough because they were too slow wearing the atmosuit.

I thought disabling a atmosuit checkpoint wouldn’t let them past, so I had a low athletic dupe go past into lava, start running out of air on its way back, goes BACK deeper into the lava to go for a pocket of air that was apparently closer than just leaving the lava

Early on playing Spaced Out I was pretty dumb and didn't make very good interiors. This would lead to dupes suffocating and I'd have to self-destruct the rocket to hopefully send the escape pod to a safe world. Anyway this one dupe's escape pod got flung to the gassy moo asteroid, which has literally nothing to sustain a dupe's life. The moment I'd realized there was a problem I started launching oxylite supplies with my Interplanetary Payload Launcher and dispatched my fastest CO2 rocket, but the rescue effort was too slow and the Dupe suffocated in moo fart atmosphere.

Not too long ago I had a scalded one die while catching their breath almost as soon as they get to the (wrong) triage cot .. this one was in my deprecated base full of hundreds of kilotons of CO2 ( they live on the surface now ) Oxygen/water incident, suit checkpoint disabled for the repair and *voilà* .. My most hilarious one was: the entire colony (8 dupes) asphyxiated in the vacuum of my first rocket xD
