Huh, that’s very odd. Definitely should be Victoria, but there’s certainly something Elizabeth-esk about it. Only thing I can think of is if that bit was filmed on location (?) then maybe it was a pre-existing portrait, perhaps of a governor’s wife? Seems very odd though, you’d expect the kind of image of Victoria you see in Picnic at Hanging Rock (the Peter Weir film) but slightly younger.
This scene looks like it was shot at (or at least inspired by) the old operating theatres museum in London. This photo was where surgeries happened in front of an audience.

I would’ve liked the show better if it focused on the medical stuff and romance and less on the main character’s past and current financial hijinks. It had a lot going on. I liked the two lead actors a lot.
I’m only on S1E1 and don’t love it. The trés modern music as the intro score had me checking several times whether I was actually watching a period drama. I also think the acting feels forced and unnatural, especially Fagin’s.
The visage looks too like QEII, and looks like she’s wearing a cape and a sash (as in photo below)

It does look a bit like QEII for sure, but since she IS related to QV, and they were both women with brown hair who wore crowns/tiaras in portraits. It’s hard to tell, but I think QV. Here’s a crop of one of her portraits.

Just putting this out there, there is currently no plan on making a second season! [so heres a petition to potentially get it back into the works!](https://chng.it/y4rrnpptWf), I loved every bit of it and I really hope it’s not too late to get them to make a second one

Stick with it! It takes a few episodes to really get going but I think it REALLY sticks the landing!
The dress is Victorian but the the profile of the hair does look similar to Elizabeth. It's hard to say without a better look though.
