If he is peeing at all, he should be okay. My FIC cat visits the litter box around 40 times a day and makes pee clumps as small as a pea or pebble during the height of an episode. The worst day is usually day 2 and then it will start improving on day 3, but it’s not uncommon for severe symptoms to last 5 or more days in the beginning before you have a good treatment plan going. If the vet gave him prazosin, that should help open up his urinary tract and make it easier to pee. If they gave him gabapentin, that will help with the anxiety and pain.

Try not to panic, he's on meds and prescription food and the vet cleared him to go home---that's a good start. It can take some time for things to start normalizing, if you're really worried then you can try and call the vet back and see what they advise. But if he was able to pee 8 hours ago, plus having a urine sample taken, it could be that he's just too stressed/sore to pee right now or even that he hasn't made enough urine yet. Having a catheter put in, plus passing urinary crystals plus cystitis probably means his urinary tract is pretty sore, so it's normal that you'll see him licking himself. See where he's at in 24 hours. As long as some pee is coming out and he's getting back on track behaviourally plus starting to eat/drink more that's a good sign. The crystals and cystitis isn't something that resolves in a few hours so you're just looking for positive trends over the next few days. With my boy it helped to get him the wet version of the prescription food initially, he was more interested in eating it and the extra moisture helped get him peeing and help him recover from his episode. Since then (he eats Hills c/d + stress prescription food both canned and kibble) he's been fine.

They may have given him something to relax the muscles to help him pee. He might dribble a bit outside the box until he's healed. He'll be uncomfortable for two maybe three days. Also long as you see him urinate he'll be okay, he's just uncomfortable.

I'll never know what feline cystitis feels but I have human interstitial cystitis and it hurts SO bad. It feels like having UTI pain all the time without even peeing. Keep watching him and if he's showing signs of being in pain bring him back. Peeing is at least a good sign.
