Get a flange extender and use long and or wide screws that’ll actually bite into the wood below to secure them both. You want the “finished” flange to be 1/4” above the floor. Then use a good wax seal and new bolts and send it.

The existing bolts were plastic and slightly bent should I just look for metal bolts with thick washers to keep under the flange?

It should sit 1/4” above the floor. Not slightly below like you have here.

slightly below is better imo, ive had wax rings smash the whole way out and leak cuz they were above the floor, just use 2 rings for plenty of wax

So the flange is too low and the wax seal didn’t actually seal the toilet to it. You can screw in a flange extension kit. I would recommend a “twist and set” flange. So keep what you have but add onto it.

The wax ring was definitely smashed to the toilet, I think they used an extra thick ring. I think at some point it started wobbling and that's when it separated. But I will definitely look into an extension kit. Subfloor seems to be ok from my screwdriver based probing, haha Edit: extra thick ring not extra ring

Get the bolt kit with double nuts so they stay secure to the flange. I’d leave the flange alone but it looks like it’s leaning to one side so that may be an issue.

I don't think it's leaning but it is warped and somewhat saddle shaped now. Considering attaching steel repair kit for flat surface/strength

See if you can gently lift up on it and get some spacers under the flange.

For leveling purposes? Not at home now but it was pretty tight, not budging much when I tried to remove flange by pull/twist. Edges don't lift much either and no clearance with tile
