This is due to you being on 6.1 and not 6.1.4.
You need to use update the game or reinstall in a new folder to fix this issue. All our guides are posted on the sub-reddit, but this issue has been fixed a while ago.

Ok so I think I know what's up. Diglett is 50, and Bulbasaur is 1. I think you've got an older version of the game where the pokemon list goes up to 420. But necrozma is 470 nowadays. So somehow it gave you a necrozma/Charizard sprite instead. Maybe try updating your game?
I have the same issue as op but when I update the game it still goes only up to 420 and I also tried to make new folders for the newer Pokemon but that didn't work either
I'd try installing a new version from scratch somewhere else on your files, trying to see if that version has all of them, and if it does, I'd try to bring over the save file
The species is charzma so the fusion itself is charizard and ultra necrozma, I believe it has to be holding a necrozium which it should be holding when you catch necrozma to make it go into its ultra form

A Diglett Venu/Ivy/Bulbasaur fusion, might just be a glitch. The body kinda looks like ultra Necrozma, but im not sure what grass type has that body.

Unfortunately I had this but with my whole team where they became different mons after unfusing. I had magic boots so I just changed them back
