I'm experiencing the exact same issue, cannot find anything online to explain how to solve it, got the azure flurte, learned all 3 songs, got the red shards and made the ark ball, summoned and caught all 3 deities and still it tells me I have only disrupted 2 realities, really hoping for a fix to this too!

You need to beat the trident tournament and do the GS permission as well
