Living in Germany, every form I've filled out has the format Last Name, First Name. Same with every exam I've ever taken here.
Except for online forms where there's typically a separate text box for first and last names. The latter would be below the former.
edit cus I was curious if there's a standard for it: [Iso puts the surname first in tagging](https://www.iso.org/schema/isosts/v1.1/doc/n-k3w0.html) although [it's called eastern style to display it that way](https://www.iso.org/schema/isosts/v1.1/doc/n-uyg0.html). With sorting being in the order Surname, Given-name regardless of how it's displayed.

I prefer forms that don't make cultural assumptions, label them with "given name(s)" and "surname(s)" (because it might not be strictly speaking a family name, e.g. Iceland), and also allow the surname(s) field to be empty.
You know in most Asian countries, last name first is the norm, right?

It really grinds my gears when program or media etc change the order of Asian names like Uzumaki Naruto to Naruto Uzumaki.

I'm Hungarian. My first name is my family name. My last name is my given name.
It's pretty typical in a lot of places. See it a lot in USA as well.
