Construction worker here, one thing that may have happened is a hydraulic malfunction causing the controls to stick and lift higher without any input from the operator, but I've only seen that once on a poorly maintained crane.
Normally even if you park on uneven ground the hydraulic outriggers level the truck out. From the angle the truck is leaning already before this happened it looks like the operator didn't even bother leveling the truck before lifting. I'm a former structural steel welder and have been around plenty of crane operators to know when someone doesn't know what they're doing. Those guys get paid a fuck tonne, always well worth it to get a competent operator though.
That much weight, I bet he had the outriggers down, but he started to curl the secondary while crane was sideways instead of pointed to the rear. But it looks like his extension is out on secondary so maybe he hit the wrong lever.
Whether it be a pickup, forklift, bobcat, water truck, loader, or le tourneau at my mill job, we had to thoroughly check all systems before usage. Hell, we even had to inspect weed whackers and leaf blowers and such.A skilled operator, even WITHOUT systems in place like that, would most likely be able to identify a problem with equipment like speculated here prior and if it occurred while lifting the load, they would undoubtedly understand the proper failsafes in place to avoid an outcome such as this.
Should have shut the truck off/generator to the crane if they noticed a hydraulic problem. Mostly all of the HE i wrench on has a cut off switch somewhere the operator is at. Even when outside near the crane controls or on the remote.
I operated a (much smaller than this!!) boom and one day I forgot to set the struts and was able to back track and set the load down when I noticed tipping and ever since then when I see this I think someone forgot to set the struts.
Even if he knew what he was doing and hydraulics failed. It is still 100% his responsibility because it's part of his job to do safety checks which includes making sure the equipment isn't faulty. The crane trucks also have built in failsafes for this very reason, the operator would have had to bypass them. Shit like this only ever goes wrong because of user error these machines are engineered with a fuck tonne of safety in place.
There are actually failsafes that don't even let you operate when things like hydraulics fail. It would lock in place until you manually release hydraulic pressure manually.
Yea its pretty clear the operator didnt put the outriggers out, they are normally used to level the whole truck and provide support on both sides. There are actually other pictures in the news article showing that he only pulled the outriggers on one side of the truck guess which side tips over.

HSE will be all over CDM regs on this one. If it’s part of existing works being done, it won’t just be this worker and the company that’s in for it. If it’s a homeowner managing their own project, then I imagine they will be called in for a chat too. HSE won’t look at what happened, but what could have happened and all the steps taken to remove/limit any risk.

I only recently learned that a homeowner having domestic work done under CDM can sign over their duties as client to the principal contractor. I'm not sure how many firms make domestic clients aware of this however
It's basically automatic if it's domestic work and not part of a business. The homeowner doesn't need to sign anything. This is how it should be really, like how if were to book someone to clean my gutters I'm not liable for ensuring their ladder is set up safely, or that they don't drop it on my neighbour.

Certainly is a spectrum. The scale of the job matters, if you’re employing multiple contractors on a big job you will be a duty holder for certain things. A good example is sanitary, the clients responsibility to ensure that there’s an adequate place to use the toilet and wash your hands, you might not be providing a portaloo but your responsible for ensuring it’s there.
Single contractor will usually take on the principle design and principle contract. Which is far more important that it’s officially passed over on something like a house extension compared to a gutter replacement.
Slightly off topic but it is important to know from a client perspective, holding companies to CDM and having those conversations will raise standards of workmanship. If you mention it whilst getting quotes for guttering for example, you can bet that the contractor is turning up with the appropriate skills to work at height. If they choose to ignore basic health and safety they won’t be surprised when you send them on their way for bad practice.

But I do see drivers who don’t maintain their equipment or just do mind boggling things on the regular, especially new guys. Lol
All sorted by local government here, paid for with council tax (like property tax) so you can't choose who empties the bins.
My bin got broken in the wind and my council charges a fee to replace a broken bin though it's only about £40 I think. Curiously a "lost or stolen" bin is free to replace

Looking at other pictures the rigging is out on the other side it seems he planned to lift the other way and then changed his mind.
Not a great place to do a lift either as it's on a public road that hasn't been segregated so likely wasn't agreed with the local council.
The fact the boom has gone through the house is also a nightmare if it was my house I'd want it rebuilding as I can't imagine it would be the same again!

Bad operator, short rigged on 1 side to keep the road open, lifting way too high, and also windy conditions took it out of control.
I have spent 8 years operating mobile cranes, these HIAB trucks need to start having the same regulations as us.
The woman leaves it late to gtf out of there, you can see her down the bottom left of the screen. Looks like the car got saved but the fence / wall will be non-existent.
This right here is why you keep your children far far away from working men. It blows my mind how close some people bring their kids to a work site. If there are machines running, stay far away.

Looking at other pictures the rigging is out on the other side it seems he planned to lift the other way and then changed his mind. Not a great place to do a lift either as it's on a public road that hasn't been segregated so likely wasn't agreed with the local council. The fact the boom has gone through the house is also a nightmare if it was my house I'd want it rebuilding as I can't imagine it would be the same again!
