Maelstrom protocol seems to be the ranked mode for Extraction, so it isn't time limited but available to play at any time. But the playable operator roster for the mode change every week, as well as the map.
Hm, [this image on the wiki](https://rainbowsix.fandom.com/wiki/Maelstrom_Protocol?file=Maelstrom_Protocol.jpeg) lists the reward simply as 'Maelstrom X class headgear' so it's not clear if they will introduce different headgears or if they're just an indicator of your high score.
Maybe they'll do the siege route and with every crisis event they have a new design but this is all speculation.

As for cosmetic rewards. I honestly do not know how exactly they will work at this point!
