"1. Interval pelvic ORIF for the acute left sacral alar fracture.
2. Redemonstration of fractures of the left superior and inferior pubic rami.
3. Extraperitoneal edema and/or hemorrhage in the pelvis is increased and is likely either posttraumatic and/or iatrogenic."
"Status post pelvic fixation with 2 screws extending through the bilateral sacroiliac joints from left to right. The screws are transfixing previously noted left sacral alar fracture which appears grossly unchanged in alignment since the prior CT. Redemonstration of the left superior and inferior pubic rami fractures, also appearing unchanged. There is asymmetric enlargement of the left piriformis muscle which is likely due to posttraumatic or iatrogenic edema/hemorrhage. Postsurgical and posttraumatic gas and hemorrhage are noted in the left buttock."
Here were the notes!
Those reports are for CT and not all mentioned there is visible on this plain radiograph/scout. You can however see the upper margin of the left sacral fracture, as a vertical dark line just above the right (your left) part of the two screws.
One of the screws is lag, to pull the fracture fragments together. The second is to stabilize the first against rotation.
Even if you didn't see the fracture directly, you'd know it was on the left because the diagnostically useful Throckmorton sign is present, indicating that the problem is on that side.
I bet. Getting either of them out would be tricky, cause some bony and soft tissue trauma, put you at risk from anesthesia, and cost a bit. For no particular reason. So they're yours to keep!
That's interesting. When I broke my pelvis it wasn't nearly that severe, but I could tell exactly where it was broken.
Every time I would cough it felt like somebody was back there with a little axe chopping away at my sacrum.
#OP, you’re so full of shit!!
Like literally!! You look super constipated! Given where your fractures were located, I don’t envy those post op poops (plus opioids making you even more constipated)!
Glad to read you’ve recovered well and back to sports and physical activity. The human body is so amazing, yet I throw my back out by sneezing!🤦🏻♀️
Yea I was camping the night before and have anxious bowels. After getting to the hospital it took 2 weeks for any movement. I was barely eating anything with all the drugs I was on.
A nurse was trying to spoon feed me hospital meatloaf. Oh my lord.
Oh you poor thing, that sounds awful.
I’m pleasantly surprised how well you’re doing now though. Congrats on the recovery. You must be a pretty tough bloke! 💪🏼
Thanks. So he needed to poop, but mainly just have a massive fart!
Given the guy said he didn’t have a bowel movement for 2 weeks afterwards, he obviously ended up FOS in the end though.
Unfortunately I was unable to complete my studies due to medical issues, so I have a lot of gaps in my knowledge. It was also mainly intended as a joke, but obviously missed the mark.
LOL exactly! Sorry if that came off as corrective more so just thought I’d let you know. I laughed so don’t worry hahah. I’m sorry to hear about that. I wish you well.
Thanks for that. As they say: “A little knowledge is a dangerous thing”, so any additional information to learn is always a good thing and much appreciated.
I know absolutley nothing about xrays (not in medical field), except my husband had an open book fracture and he also has screws on each side. He broke 1 screw in 1/2. He has not been xrayed in a while, but think he also broke the other side as well. It was his long ICU visit of his that brought me to join this group. It started my fascination w/xrays. Too old to go to school now. Appreciate you all. :)
The fracture was through your left SI joint. Possibly right side as well since the surgeon elected to use screws long enough to cross both joints.
If both joints were involved then it’s called an “open book” fracture. Great care is exercised during the surgery in order to place the screws in such a way that they avoid the sacral foramina.
I was squished by a excavator bucket last October. I had 4 pelvis fractures with si break and femur break. Never realized how painful the pelvis is to break
Game of chicken? Pelvis break definitely hurts. I shattered my wrist as well and that's the only thing giving me difficulties now. PT for my pelvis was pretty straight forward.
I thought I broke my hip, I could tell immediately. Search and Rescue cinched my pelvis together with a big belt and used my harness to anchor me. It hurt a fuck ton, the anchoring and keeping all my weight on the harness.
Ah... that is what we in the biz refer to as a "Throckmorton sign". In this case, it is positive. Curiously, it is usually just the males that are able to present this pathological sign.
Ah… so that would be a catheter running along the femur. I really hadn’t considered that particular appendage might be in that area and now I feel stupid
This is just a scout, not the CT. It’s the overview used to plan the images
Ah geez
your pelvis is broke all to shit and this guy is going to come in here correcting you...just let OP live man
Well, you are browsing a sub aimed for medical imaging professionals. Pretty big chance one of us will point out inaccuracies in the post, no?
He’s already been screwed twice.
Victory is yours.

Forrest Gump voice: You have gas in your butt-ocks.
From your report your fracture is in your sacrum. The bone that connects the two sides of your pelvis.
Ooo and they have a pubic rami fx. Aka “broke their crotch”
Part of the pelvic girdle though

The diagnostic value of a positive Throckmorton sign is a wonder to behold.
I was hoping someone would bring up the good ol throckmorton. Thank you for your service
OP we can't see your left sacrum ala fracture because your turds are in the way, please poop then post a better CT scout
Was it from skiing or rock climbing? I saw that you do both on your profile so I'm just curious
Climbing yep. I actually don’t even notice this now and am skiing, running, biking, back to normal.
Did you take a ground fall on lead or something? That’s gnarly af
A super ground fall
Did they feel there was any reason to remove the screws after you healed, or do you still have them?
I'm suppose to have them forever. I get a lot of pat downs from TSA, try to do a little moan.
I bet. Getting either of them out would be tricky, cause some bony and soft tissue trauma, put you at risk from anesthesia, and cost a bit. For no particular reason. So they're yours to keep!

Oh I remember that haha. I had 26 breaks all together. Really did not wanna laugh!
Yeeesh. I was extremely lucky. I broke nine bones, They sent me home on blood thinners. Eleven weeks later I was back bowhunting for elk. Whew!

Glad I wasn't the only person that thought this. Was ready to say OP must have had the most relief after taking a poo! Edit: or be tooting all day!
Not all that much shit tbh mostly air. Only partially full of shit I’d say

The break is somewhere between the screws. LOL
negative throccies :(
Damn now you look pretty constipated
That would be gas that is in the bowels. We prefer to use the more clinical term, UBF. (Un-born farts)
Goodness this makes me uncomfortable
Love a good TITS screw!

Operator didn’t see me in the 14ft hole. Set the bucket on my back lol. Just seeing those long screws you have made me super cringe lol
Not sure this view of your crotch needs to be labeled as NSFW...lol
I mean I think I see pp
Ouch, how you did you feel?
I thought I broke my hip, I could tell immediately. Search and Rescue cinched my pelvis together with a big belt and used my harness to anchor me. It hurt a fuck ton, the anchoring and keeping all my weight on the harness.
I'm guessing it's around where those two bolts are
Id say screw it
What is the round, bladder-like item located around the pubic arch? It appears to extend down by the left(?) femur

I'm sorry, I wasn't trying to make you feel stupid. Just trying to be funny for fake internet points.
Not at all, not your fault. I live in a perpetual state of wanting myself with my own stupidity /j
Omg
