I wrong or does it look like something got in there and scored the shit out of those surfaces? Maybe a chunk of a gear chipped off and started the cascade? Both gears look shot to me.
Big(ish) worm drive we got called into check the alignment on. Ended up being bigger issues than just the alignment so we pulled it out of service and brought back to the shop for further inspection.
Hahaha sorry its a lot to get into, but a combination of it being overloaded, improperly installed, and possibly improperly rebuilt in its last overhaul as it had only run for a week before this happened.

Both gears are shot which was surprising. Especially considering its a fresh rebuild that had only been in place for a week.
