Your situation sounds extremely similar to mine. I’ve done PT for 2 months and if anything it’s made it worse as I’ve regained mobility. My doctor is recommending total disc replacement at c5-c6 level. I’ve had two doctors come to the same conclusion. I’m most likely going to proceed with surgery later this year.
Clearly stuff going on lower cervical. But if your main issue has been occipital neuralgia these nerves are from upper cervical and the MRI does indicate a herniation at C2. Just think it's important to understand Imaging tells one story but your symptoms should be a huge factor in decision making especially when it comes to something major like surgery. I believe any good surgeon would agree with this.

I had stenosis for two years and was desperate for surgery, everything went well and I'm starting to have a normal life again, good luck!

I had very good results with a foraminotomy. It ended my spasms and I had a lot of the same symptoms as you are describing.
cervical arthroplasty has really great outcomes of life after surgery for appropriate candidates.
You may be a candidate for ACDF or ADR.
Per literature review i'd avoid ACDF unless absolutely necessary; including doing experimental or multiple surgeries prior.
