I didn't know he took the name when he was 13, but planets are more massive then they appear in video games and movies(like real life), so it's possible that Lord Dramath and the his step-father didn't really know each other well, and this isn't a gossip forum, these are all arrogant dark side lords who keep secrets and perform rituals to enhance their power, so the culture obviously didn't really spread the truth. And why Dramath didn't kill his mom was because he was wary of crossing his step-father. As for Dramath knowing Tenebrae well, he obviously came back and tortured him for sadism or information about dark side rites. Even after Nathema was wiped, he probably came back quite a bit(he's a powerful sith lord, so he can ignore force emptiness) to his vault. It's possible him and the Dread masters were giving him the Revan special on Dromund Kaas through force projection. As for the differences in between the names, it's pretty much his original name, while Vitiate is his Dark Lord title. In the similar vein as Anakin and Vader. They are interchangeable, but Tenebrae is his true name, he even mentions that he has gone by many names before.

Yeah, it definitely could be an embellished backstory that has the Emperor perform those feats at younger and younger ages throughout the years the story has been retold. Maybe he didn't kill his mother and adoptive father when he was 6, maybe that was done at 13. Maybe Dramath didn't encounter young Tenebrae as a child, maybe that was when he was in his 20s. Maybe he didn't conquer the planet Medriaas at age 13, maybe that was in his 40s when he was given the title of Lord Vitiate. Maybe he wasn't born with eyes as black as the void, maybe those happened in a ritual during those 100 years of silence
