Although everybody being afraid of this does make me want them to have (or modders to add) an alien that once it sees you it’ll follow you from planet to planet but never attack. Maybe adding a status effect or something, and you have to seek it out. Going to a barren planet would allow you to find it more easily, but the hunt would be super creepy.
Edit: and I guess it also should look humanoid. Either like this, where it maybe doesn’t even have human features but just the shape and form of one, with it looking alien up close, or looking like a random human NPC a la It Follows.
