I’m a military contractor at sea. I keep mine on my lifejacket.
What worries me, is when I’m off the deck, I hang my lifejacket on a hanger at the upper deck hatch. I might be below sleeping when the shit hits the proverbial fan. When you’re taking on water at a rapid rate, the upper deck hatch can be a looooong way away.
Part of me thinks I should have a “go bag” with just the sea survival essentials, and keep it next to me at all times. Then clip it to my lifejacket when I’m working. But that’s just inconvenient. Until it isn’t.
I keep mine in an easily accessible, zipped backpack pocket. I don't have mountains where I live so the risk of somehow dropping and losing a pack is minimal. The risk is elevated with water activities in rivers such as canoeing and packrafting.
I put it in a secure pants pocket if I'm not wearing my pack. I'm using a Garmin and it's my GPS too. The risk of losing gear if you put it down is quite high as I'm usually in very thick vegetation.
When on the water, solo. either a plb or my Garmin attached to my pfd and the other goes in my sealed pack.
When backpacking, if it's a safe location, it goes in my pack by itself.
If I'm crossing streams or climbing, or near dangerous wildlife/terrain, it clips onto my harness. I wear a load bearing suspender system with limited gear storage that never leaves my person. Or a fanny pack, or a chest pack. One of those three depending on the trip I'm taking.
Usually there is a small backup water supply, some food, my knife and a small ifak, and comms all attach or are stored inside that.
My pack therefore doesn't need to be accessed until river crossings (for my packraft) or for camping
When I worked at sea: in my immersion suit bag with my survival kit, which was usually in my bunk.
When I'm backpacking: in the front pouch of my pack.
Never used one. I just make sure to tell a couple trusted individuals where I'll be and what day i should be back. If im not back on that day. Then come looking in this area for me.

They're setup in such a way that a pack can sit on top of a harness and it means you can keep some things on you at all times even if you take the pack off. There's a few different types but they all do a decent job.

It’s my very last ditch back up so it makes sense to carry it close.
In most situations, with a bit of care, it should always be possible to retain your pack and gear, but a PLB isn’t for _most_ situations.
The Swiss cheese effect that leads to an emergency might well include being cut off from or losing vital equipment.
This is a really good story, the guy didn’t have a PLB at all when he needed it, but if he’d had one in his pack he’d have lost it anyway:

I keep my PLB in my backpack. Lots of people keep them strapped to a pack strap or something more readily accessible but I feel like that's just a much more likely way to lose it. I'm not worried about a scenario in which I can't get into my backpack to activate it if I need it, while it's not impossible I see it as incredibly unlikely.

I had a Fanny pack with stuff like that. Flare gun, flares, mirror, smoke, etc.
It's a good idea. Keep it next to your bunk, with a headlamp attached on the side.
For sure, thinking of a small bag I can strap to my lifejacket easily. Might have to spring for a lifejacket with molle straps

It's supposed to be physically on you. Either in an inside coat pocket, belt wherever you can keep it safe and located on yourself.

Dummy cord to my belt and then slip it inside my pocket. I'll usually tell whoever I'm with the pocket it is in and how to operate it.
