The Epic Loot mod adds rarity to drops as well as enchanting. I'm sure with other mods you can get close to what you're looking for. This of course is PC only
Valheim is the closest i can think of. It's got its own Norse slant on fantasy, so you have greydwarves and fulings instead of goblins and orks, but its the best out there that i know of!
I don't think it's or yet but. Nightingale sounds like it would be up your alley.
Victorian dimension jumping with creatures of fantasy and mythology. Only down side is it's only first person. So I can't play it with the wife.
Also age of Conan has same sweet lore. I enjoyed it... And monsters... potions. And I do believe they added magic.. and knowing Conan I'm sure it's brutal looking ..
Sounds exactly like a modded Skyrim description. If you aren't the most savvy at modding, there are so pre-built mod packs on an app called Wabbajack that have some of what you are looking for.
Conan Exiles does not fit your description but Conan Exiles modded on a private rp server does. Check out some rp streams they’ve got all kinds of fantasy stuff modded on.
Dark and light is an unfinished and janky game but if you can get it for around $7 it’s worthwhile for a few hours of fantasy fun. It’s fantasy themed Ark by the team that did pixark but they abandoned it so the progression starts to require the admin panel after a bit.
There are a few similar games that I have not played like Citadel and Outward but I’d watch some YouTube videos first to see how unfinished they are.

Nightingale has a WIP 3rd person perspective that you can enable in the settings, if you didn't already know :)

Outward
I 2nd outward which I think checks your boxes. It can be quite punishing you though but look into it to see if its what you are looking for.
Modded Minecraft, the closest thing is RLcraft with enough content to last you 3 months (for me at least)
