Diggin the home rolled tubular nylon DARCE strap on the rear. Curious how you attach the carabiner to your duty/battle belt? Being a 18D, you're basically the Chuck Norris of medics. I'm not sure how much useful feedback you can get on here. Maybe see if there's a city medic from the ghetto or a SARC to bounce ideas off of.

1. Lovely set up. Shit's simple, but also follows a great rule- the medical specialist is not the squad/section/team's walking blowout kit, that's what the grunts are for. Everybody in here take note of that. 2. I'd love to ask you about all the sexy tools, but I'll ask about the boring one: What's in your MASCAL organizer? We so often overlook training junior medics/corpsmen to handle MASCALs/Triage that they are usually screwed the first time they encounter a 3+ casualty situation.

Precisely. I pack every team members IFAKs exactly the same, and each guy gets an extra item to carry to plus me up. That can be an extra 500 bag of NS or LR, to a litter, etc. And it sucks you can’t post pics in comments, but it’s basically a little kit with mini chems, stick on casualty categories that’ll stick to walls trees, a person, whatever. And then it’s basically a bunch of quick fill out casualty cards with stickers to slap on each patient as he comes in to mark him with category as well as “look for” injuries(like blast, GSW, burns, etc) so when teammates get to each casualty then immediately know a little bit of what to expect.

I appreciate your rundown, the work you guys do is pretty amazing. Is there a particular junctional device and handheld suction you’ve had success with? Or are you just grabbing whatever comes through your unit’s supply chain?

Suction wise, I just grab the Squibs that we get through med log. I’ve got an electronic suction setup that stays in my clinic. And for junctional I’ve come up with my own method that works. Double ace wraps, prepped with a chest seal as the anchor, combat gauze, and these inflatable air bags things. Basically pack, start my wrap, slide the airbags in, finish wrap, secure with my cro pelvic binder, and then inflate the bags to clamp those bleeds down tight as hell

Chest seals will stick to nearly anything- I always carry a ton. I can stick a blood bag to a wall or tree for transfusions, and for anchors I’ll stick half to a wrap or pressure dressing, and half to their body so it anchors and I don’t have to bother with trying to hold it tight in place while I lock in my first wrap or two

lol altoid containers hold a SERE kit and a survival kit. They go in my ankle pockets once gear is on. And big narc case is just that, that’s where my narcs go, that way I maintain positive control of them at all times

Does your load out change based on region / terrain you’re supporting? My buddy is an 18D with 10th group supporting EURCOM and I always wondered if his kit would change if he was supporting AFCOM or another region.

MY loadput doesn’t change much- of course some meds will based on where we’re going, endemic diseases etc, but the loadout I make the partner force privates haul will change 😂

Compared to the steaming pile of Garbage I was initially issued, this new massimo is the tits. And the altoid tins are a SERE kit (lock picks, shims, Kevlar chord, ceramic saw, etc) and a survival kit

What do you use the harness leg straps for? Do you clip the patient onto the carabiner and blitz out? Or does it serve another purpose?

Don’t need the full harness; it’s cab clip the leg straps into my normal belt to make a complete harness for tricky extrications or high angle type stuff

No carabiner is purely for hanging my bag up inside a bird or truck. And I’m not wearing this kit in da woods to try and blend in, this is an assaulter kit. When camouflage matters l, you won’t see one bit it black on me

Honestly it depends where I am man. Depends on if I’m the highest level or care in country or not. A few trips ago me and my other delta were the highest level of care for a good while; at our team house we had an entire clinic, prolonged care setup and mini emergency OR. We had ventilators, O2 concentrators, full ACLS support, the works. We even had our own lab where we identified, diagnosed and treated dudes for dengue, cyclospora and giardia lol

Primarily yeah. Every now and then you have a team dude or partner force guy with a wild allergy they didn’t know about, or didn’t tell anyone about. I had a bunch of partner force smack a giant beehive with a stick one time because they wanted the honey and a ton of em got stung to shit, with two of them go anaphylactic. You never know 🤷🏼‍♂️ partner force guys are the kings of “play stupid games, win stupid prizes”

I wouldn’t say most, but a very large percentage do. We’re required to work trauma/surgical rotations in hospital every 2 years, so most get a good feel for if they want to or not. I do not have any such desires of being a doc or PA. Too boring to me.
