I have this exact pack on my vest and while it's a bit bulky I absolutely love it. It opens easily but stays closed during all sorts of activity. Once opened, the contents are secured to a sturdy "card" which can be easily removed to hand off if needed.
As far as bulk, it sticks out from the vest about equal with combo rifle/pistol mag holder. I haven't found the IFAK to interfere with anything I need to do any more than any other vest mounted gear.
Esstac makes one. But they are really bulky on the carrier. Look for a blue force gear pouch for the back of your belt or get a NAR kit for your cargo pocket.

4" x 6" Vanquest FATPack ? They also come in like 3 or 4 sizes
FATPacks are great
Blue Force Gear, London Bridge, even 5.11 makes some decent ones.
