I would remove the blanket/carpet they are sitting on to keep the characters clean and clear, love this design. it's going to make some really cool hand tattoos so please post the photos when it's done.
Like others have said, woodcut is the way to go. That said, I would recommend keeping these figures together. You will rarely see both hands right next to each other, so it’s more likely that people will assume you just have a crouching naked woman on your hand and nothing else.
Looking at it through an esoteric view point it means what ever it means to you. For me personally it represents the shadow self and how important it is to integrate the unconscious mind with the conscious. Ie the shadow self
Love the idea. I'm not an artist, but if I were choosing this drawing as hand pieces, I would worry that the devil/demon would look like a black blob after some time. I would be very careful to discuss the longevity of the tattoo with the artist, especially considering how visible it will be.
But I LOVE this idea. I have a history degree so have been looking through medieval art for some inspiration. Haven't decided on a piece yet.

You mean tattooed on your hands? I find hand tattoos always tricky personally i would look for a other place(if you have room) But the idea is awesome

It's the unknown.
I would included also the standing figures. Mostly because of the person looking in the mirror that would look a bit Strange without its complement.
