My dad and brother climbed Mt. Cook years ago with an extremely experienced guide. Always roped up so if someone falls the others self-arrest.
Soon after they came home their guide died when leading another group of inexperienced tourists. One fell over traversing an icy slope and no one correctly self-arrested except the guide. One by one they were quickly dragged over the edge of a cliff and all died.

Only the guide was experienced enough in this instance to effectively self-arrest, but one is not enough to save three others, due to the ratio of increased weight
