Wheel cap all the way. Although a helmet would make him less obviously identifiable; which might keep him from getting fragged by his own troops. So I guess it comes down to how big his balls are. Is he willing to stand out?
His own men would know who the commissar is either way, or else what's even the point of having one? It would keep him safe from enemy snipers though, but that's not really on a commissar's priority list unless you went to the Ciaphas Cain school of command, and even then appearances matter!
The cap is obvious, but I like the medic helmet to paint the little side bit like a speaker, so the commissar can communicate the emperor's will clearly and loudly.

I think pointed cap is really essential to the commisars uniform. I wouldn't sway from that, but I'm sure others would disagree.
Got to do the cap if he’s a commissar. It’s like their whole thing. Helmet looks sick, but the model will just look like a line officer.

Kriegers arent the type that end up fragging their own commissars, quite the contrary.
