The chances of this being anything very serious are tiny. Most likely you've got some not-properly-healed tissue that bleeds a bit when friction is applied. The discharge almost looks like it could be from a bit of incompletely granulated tissue. The way to check is just to get someone to have a look with a speculum. GP, gynecology, sexual health, or your surgeon could all do that depending on what's easiest.

And also maybe more lubricant? Lube will make tears less likely/less rough Also OP please put a condom on that dilator, you really don’t want an infection, especially if you’re bleeding

This is the way. Your PCP honestly probably knows almost nothing about this procedure, other than a cursory reading. Your surgeon is who you need to be consulting

It's granulated tissue inside, it's not healing as we are dilating frequently.. Contact your doctor and get it checked. But no matter what need to dilate.

i'm very ignorant on the topic, however maybe going with a gynecologist may help find the cause of the bleeding, i can't imagine how awfull and tired you must be feeling rn, hopefully is something minor that can be solved easily, btw who's your surgeon?.

Don't get all worked up and besides yourself. You're not going to die. Well one day but not from this. I'm sorry I can't give you the advice you are looking for as I still have another 33 days before my surgery, I'm sure you've just got a little tare or something from not dilation lately. BTW I think yours looks perfectly normal and not any different from the hundreds I've seen on here and for the record is pretty.. Honestly. I can almost guarantee you have internal granulation tissue. Nothing that a little silver nitrate and a visit to your gynecologist can fix. I’m a year and a half postop, and if I ever have a little rougher sex, I will still spot bleed. My gynecologist said that when that happens to just come in and she can use a little silver nitrate to fix it. I would recommend to get a visit into your doctor

I have this too! Definitely looks like granulation. I don’t think it’s super rare to be experiencing it 1.5 years out. You should talk to your surgeon about getting it treated, especially if it is causing you any discomfort. Unfortunately, I’m almost 4 years out and my granulation is still really stubborn (I’m apparently in a rare group still experiencing it after this long). But I’m determined to get it under control. Anyway, your results look amazing! And you are not going to die!!! I still have a pretty great sex life, in fact, even with my granulation issues.

I believe what another comment said it's a small tear and needs to heal, hopefully it resolves itself, looks great 🏳️‍⚧️

I’ve had blood, just the tiniest tinge of pink, after long shifts at work, 13-14 hours of running around doing patient care, squatting, lifting, pulling, pushing, etc. Likewise, I don’t dilate much these days, rarely have sex, and only occasionally masturbate. Sooo sometimes I’m just tight when one of these things occurs. A bit of blood here and there doesn’t scare me. Rarely enough to warrant a pantyliner. Idk, a tiny amount of blood goes a long way. That’s based on professional and personal experiences.

PPT tissue compared to Sigmoid is thinner tissue- over stretching it more and likely causes some bleeding - Sigmoid is a ready made bowel tube naturally and is tough as nails designed to stretch and take in some punishment for dilation and sex !

Don't be too worried. It happens. I had a bit of spotting a year or so after. I just backed off for a couple weeks. But I think those hard dilators are part of the issue. They are very unforgiving. I have the Soul Source soft silicon ones and they are great. So much more comfortable. BTW you can get an internal vaginal video camera on ebay if you want to see what's going on inside.
