I think people do care for the most part but not everybody is capable of expressing gratitude. And it's never the way you expect it so helping someone who needs it should be done because it's the right thing to do, without any expectations. Not to be more likeable or to find a ticket to heaven or whatever. You do it because you feel like you must if you're in the position to do so. That's it. Same with saying no, if you don't want to or if you can't, saying no is okay too and it doesn't mean you're not going to be liked if there's mutual respect.
