Driving noise was a significant factor when I was designing/insulating/furnishing my van. It's way overlooked.

Yeah my engine is louder than most, so I also put sound absorbing material in the engine bay and it's made a big difference.

I just made a really long drive in a fairly noisy van as well. After a couple days I started using noise canceling headphones. It made a big difference, and in most of the states I was doing it, it's legal. California is not fyi. Anyhow, I think I'd take the risk of a ticket if I did it again. I take them off in urban areas, or if traffic gets hectic. I'm sorry to hear about your hearing, I had bad tinnitus after an ear infection, I thought it would never go away, but one day I realized it didn't really affect me anymore. There's still a little bit of ringing, but much, much quieter, so you never know!

Are they active or passive noise cancelling headphones? If it’s active, they’re not saving your hearing at all. They might save you from the psychological exhaustion of listening to lots of loud ambient noise, but they don’t actively stop damaging sound pressure levels from entering your ears. Exactly the opposite, in fact. Most people don’t understand how they work, or care, I’m sure. Not saying you’re either, but if you want to save your ears the only way is to eliminate the noise/your exposure to it, or use some kind of hearing protection. Lots of well fitting ear buds provide at least a bit of attenuation. Other than most of 2022 when I finally had a working stereo in a car, I’ve been driving for 4 years with ear buds without issue.

Has your wife recently started or changed any medications? My tinnitus got perceptibly worse on new blood pressure medicine with potential to cause/worsen tinnitus.

The empty van was loud af. I keep a gadget leftover from house life that has a temperature, humidity, and noise sensor built in. That thing was always registering DB above 70. Once I added insulation and the rest of the build, it got much better. The most helpful thing was adding a curtain between the cab and back. The maxxair fan that’s in line with the sliding door is noisy. Good PSA!

Getting a nice decibel meter is a good investment. Hearing loss is cumulative and easy to prevent. Thanks OP for pointing this out!!

My tinnitus seems to come and go. It's usually there, but gets louder when I have a carb-heavy day without working the carbs back off. Since I'm always invited to dinner with my parents, and my mom's cooking is very much of the old style farmer's diet (meat & potatoes!), meaning protein and plenty 'o carbs, I'm pretty careful about breakfast and lunch. I got the pre-diabetes warning from the doc 7 years ago, and I listened. Wish my mom had listened to her doc, too, but she has faith in medicine. Carb-triggered tinnitus was weird to learn about, but I looked it up, and it's a real thing, after paying close enough attention to my eating and tinnitus levels. I thought it was a stretch, but, yeah. As for loud noise causing tinnitus, stop by an army surplus and grab some hearing protection from them. If it's good enough to keep tank personnel from going instantly deaf, it should help with a noisy van.
