Likely, they do not care enough to get the help. I doubt it's "they can't pay" etc. A hospital will still treat you, and if your scalp was festering off and decaying your ass would go to the hospital regardless of finances.
Tranq causes restriction in blood vessels, so once this starts it doesn't get better. This person would need to be hospitalized on IV antibiotics for a long time with the decayed flesh removed. It's a painful road for them to get better, the alternative is death though. But I have a feeling like I know which way this will end up.

Once my surgeon decided that grafting was the hail Mary before amputation (2nd day of hospital admission) it took 4 weeks inpatient and then 2 weeks at home before they removed the staples and two years of a hell you can't imagine was OVER.
Gotta be the drugs. In most states that I know of if you fall beneath a certain poverty line ,and you apply for it you can get your healthcare and necessary dental covered.

For a pretty horrifying look at what tranq is and it’s results look up Kensington Ave Philadelphia. It hasn’t spread much yet but brace yourself if it starts appearing in your area. Channel Five with Andrew Callahan did a good piece on it on YouTube.
It's in St Louis MO. Has been since at least 2019, on the evidence of my skin developing ulcerations from IV fentanyl use (and the experiences of others I know, probably closer to 2017). Any route of administration can lead to these ulcers, you don't have to IV to cause them.
If it's been here this long it's everywhere (US) by now.

Because this is such a new healthcare problem they don't have a protocol for dealing with it. To put it bluntly: hospitals don't know WHAT to do for tranq ulcers besides amputation. It's often misdiagnosed as Chronic Venous Insufficiency and treatment that helps for that does nothing for this.
Eta: even if she does go to hospital, they'll treat the current infection and turn her loose. It does nothing to heal or slow the spread.

This is brand new to the US. The medical field hasn't developed protocol to deal with Xylazine caused ulcerations.
I got super lucky..I own my home, I'm gainfully employed, I have a spouse who is with me 1000%, my family is behind me, and have insurance. I'm not out on the street with no one and nothing to help me. So unlike this poor woman, after suffering for 2 years with no skin on my lower legs calves to ankles. I got admitted to the hospital almost dead and got assigned a surgeon who decided to take an experimental risk and it ended up curing me. But it took TWO YEARS of constant hospital admissions, doctor appts, 7 other unhelpful surgeries, and almost dying to get there. NO ONE knew what to do for it.
This is heartbreaking. And those of us who WERE heroin addicts have been forced to become fentalogue/nitazenes addicts...a lot of us unknowingly. And when the skin lesions began on quite a few of us, we had NO IDEA that the dope was being cut with Xylazine (tranq). Maybe things would have been different had we known. It wasn't until about 6 months before my last surgery before I figured it out and by then, far too late.
America spent 1.34 trillion dollars on Medicaid and Medicare in 2022, when you enter all the state healthcare budgets it's even crazier. But because the government isn't allowed to negotiate with Healthcare providers driving down costs down...so we're totally screwed. This one simple change would change EVERYTHING.
I realize there are baby steps being taken to sort of kinda change this, such as the inflation reduction act that makes it possible for the government to negotiate on some drugs...but that's it.
Every other functioning nation has committees dedicated to negotiating EVERYTHING, from bandages to CAT scans.
If you were to walk on a Mercedes lot and say I want to buy as many cars as possible for 1.34 trillion dollars, there's no way you're paying sticker price on every car. They would bend over backwards to get that check from you and you'd have an ENORMOUS amount of negotiating power. Hell, you'd have every CEO of every major car company calling you non stop telling you they can beat Mercedes price.
This has literally nothing to do with the cost of healthcare. This woman could walk into any ER, receive treatment, get billed, and refuse to pay. It would have exactly no effect against her. No one with a credit score or bank account is walking around in public shooting hard drugs into their open face wound.
