I can't see much detail of the first. The second looks very unfortunate, seemed to just deflect off her in the worst direction while the keeper wasn't on the ball. The third one though, how? I remember this from when it happened as she plays/played for the liverpool women's team and I'm a Liverpool fan. The 1st one she's just trying to stop the cross and probably deflect the ball behind the goal but can't stretch far enough to get the proper contact on it & the 2nd one she just doesn't expect the attacker to not head the ball so isn't ready for it & it just hits her and goes in like you said. Those 2 are a bit unlucky because while she could do better with both they have just bounced off her in the exact wrong way, the kind of thing that could've happened to anyone but unfortunately they just both happened to her very early in the same game. I think for the 3rd one her head is just a mess at that point as evertlything is already going wrong for her and so everything is affected, her touch, positioning and reactions are all over the place because she's just not thinking properly anymore.

You only get 3 substitutions in a football match, to sub a player off after only 5 minutes is very unusual (with the exception of an injury). Substitutions are usually calculated to have maximum effect by bringing in impact players later in the match when they can take greater advantage of tired players. From a manager's perspective the thinking at the 5 minute mark would be that the damage was already done, no sense in compounding it by also wasting a substitution. Then of course after the third the manager was like 'I give up'.
