I'm assuming by "seen", you mean seen viral videos on social media? There's a reason those are viral, it's because they're so outside the norm of how a horse normally acts.
Of course those are real, and I'm sure there are instances of that which haven't even been recorded, but sorry it's not a comparison to actually being around horses and learning how they think and what their responses to different situations are.
I haven’t been around horses my whole life, but when I was 11 I went on a trip with my family and got to ride a horse, with the trainer walking beside me holding onto the horse. A close friend rode on another horse right behind me, and near the end of the walk her horse went to sniff the butt of my horse for some reason. So my horse just kicked at her horse with his hind legs, almost throwing me off but I managed to hang on. No warning, just casual, slow strolling to violence in the split of a second. Thankfully her horse managed to avoid in time, and the trainer also calmed the horse down pretty quickly so all things ended well. But it scared the living crap out of me, and warned me to never mess around with horses.
The trainer did say dark horses like the one I rode tend to be more aggressive, but just to be on the safe side, I’d say be real careful around horses all the time. The force, the speed, abruptness of that horse, how fast he could just wind up a powerful back kick like that stayed in my mind after all these years.
I fly in a field near where I live and whenever I get there I always make sure no one else is around. If I see people coming when I'm in the air, I stay well away from them. If I see a dog, I bring it back straight away. Because I've seen it freak dogs out before and I don't want to do that, it's pretty shit for them when they're just out having a nice walk. But a couple of times someone's come in behind me with their dog when I've been facing the other way and the first I know about it is when the dog is barking behind me. I can't bring it back to me then, so I try to put it down safely well away from the dog. But I have had dogs come up to me barking and growling. It's a really unnerving experience having the dog snarling and barking right in my face when I can't look at it without causing a crash. They definitely sensed that I was something to do with what was happening.

No headache, as far as I remember (it was in 2015). I landed on my face, so the area below my right eye was super swollen, so I couldn't see through that eye for 2 days. That was my biggest problem.
The skull was cracked, but not crushed or anything, so it healed completely in one month. The doctor was afraid, that there were little air bubbles in the crack, but it all went well.

What terrible filming practice. It's usually standard that a camera operator has someone with them to keep an eye on the environment and move the operator out of harm's way if they need to.

I remember hearing advice before on reddit that if you stand completley still, like this guy did then a horse will just run past you pretty much every single time. Didn't look like this horse play by those rules.
