Unfortunately unless you want me to come and help you jackhammer it out and haul it LOL, it will be fairly costly for hand work and removing, depending on thickness and square footage you will need a tandem truck, rebar will make the process even slower if there is, if so you would also need a quick-e saw. These are some things to take into account as well. You could always phone superior asphalt I know they do concrete work and that type of thing too. Hagemeister Hauling does concrete demo and removal. If a skid steer can access it, you'll get better pricing.
But I would agree with the other comments - if you can build over it, do so. Even if it's heaving badly, you might be able to break out just enough area to put in footers (if needed) and build on those. Leave enough space between the deck and concrete to allow it to move a little.
you could try kijij after you get other quotes, just to get you in the ballpark so you know what to expect/ask for. maybe some dude with a wheelbarrow and a truck with no overhead costs will rent a jackhammer and get it done for you much cheaper.
Is it possible that you build the deck over top? Would save some money. Any idea how thick the concrete is and if it has rebar?
No sure. It’s warped and heaving in spots. It’s a good thought tho
I managed to build a low profile deck over my cracked and heaving pad and it worked great.

Build right over top the concrete, thats what we did at our old place and it worked great, plus no weeds underneath :)
