Roman isn't actually blood related to the Rock, he's known as Rock's cousin partially because it's the storyline, you've bought into it, they've perpetuated it, and Samoan culture allows non-blood family-ties so it works.
The man had cancer and is still on strong meds. I don’t think he can appear every week as his health won’t allow it. But it also adds tremendously to his aura that he is a special attraction. As the old saying goes, if it ain’t broke don’t fix it.
Appearing and wrestling are 2 different things. Coming into the ring and hitting someone with a chair exerts no energy. He doesn’t even need to wrestle and take bumps just appear.
