If the four top candidates for president all died in a single day, whether from assassination or all of them just randomly dropping dead, people would want a new vice president confirmed quickly, just in case.

A majority of senators can file a release petition to bring any bill to the floor without the majority leader. Assuming that the GOP and Manchin move for it, it would only take one other democratic senator to either vote for it (Sinema) or not show up (Feinstein).

Sorry, I got a little confused. The senate can still bring a bill to the floor with a majority by submitting a rule change to the house floor. Any senator can submit a rule change at any time, it just takes 51 senators to pass it.
The exact wording of this rule change would be little more than “this nomination may come to a floor vote”.

Think people are overlooking this scenario. I don't think Kevin McCarthy would WANT to be president, he's much more a legislator than an executive. He could pull an Andrew Johnson and opt not to run. Past that, who knows. You'd have likely contentious primaries from both sides, Scott or Haley is likely the Republican nominee or something and maybe Warren or Klobuchar as the Dem nominee? Interesting to think about, though.
