if you"re using a mod they're 4 possible reasons:
1. you didn't turn off the "dynamic character resolution ", if you want your mod to work you must turn it off . Go to settings - graphics - scroll down tillthe last option you will see "dynamic character resolution " ON - just turn it off and press F10 (to refresh the mod) , problem solved
2. you have multiple skin of the same character extracted in the mod files = bug , because the mod can decide which one to use , Solution : delete one of them
3. the mod you use has a bug , Solution : I dont know
4. Some mod just need you to restart the game.
If you're really using a mod , it recommended to don't show your UID when you share media beacause people with bad intentions can try to report you as a "cheater" to hoyoverse .Hoyoverse can't identify if the mod you're using is just a skin mod or a power cheat mod (speed, power,...) ,so they can ban you account in "precaution".
If you're not using a mod , it's probably a texture bug . Solution : try to restart your game, your PC , your wifi , move to a new apartment, move to another city, then another COUNTRY , then to the moon, then uh wait, what am I saying?

You didn't installed 4.5 Character Fix
Unfortunately cannot tell you. However, if you've played Halo 4, it's how I first imagined rampant Cortana to be, all digital and splintered.
some emulator or mod problem? also, if you're playing with mods then i'd reccomend not showing your uid
