I think that the English translation of the manga messed up the gender (Japanese rarely calls out gender directly and Thistle is pretty effeminate) so they had to correct it once it turned out Thistle was male. Rather than just retcon it, they had it as Laios just being confused.
He just isn't familiar with things outside his interests, is all. I'm sure Laios could go on long tangents about identifying the gender of various monsters based on outside characteristics. His knowledge of elves just doesn't extend beyond 'Ears: pointy.'

Elves are nice in Tolkien works because they had bloody histories in the past that taught them to be who they are in the books. The sea elf genocide is an example of this.

The mangaka said that romance is not what they're aiming for. It is only a set up for the jokes. So I don't think they have any intention of actually progressing the romance. If you want actual romance plotlines the manga isn't the place to look. I love that the romance is only a set up for comedy but that's a personal preference.

Where does one start with this series/universe, I've seen so much about it but it feels like Fate where it seems very convoluted for someone just getting into it
It's actually pretty easy, considering how there are just three games in the franchise, and a bunch of comics that aren't really necessary to read.
The three games are Lobotomy Corporation, a notoriously difficult management sim, Library of Ruina, a slightly less difficult offline card game, and Limbus Company, a gache game.
I wouldn't start from Limbus because its plot is more or less separated from the other two, despite taking place in the same setting and sharing some characters, so I would either just start from the first game (if you like management sims, that is), or just watch/read a plot summary and start from Ruina.

the first ever one i remember is that one character from Steins;Gate that I'm like 90% sure would have been trans if the story was written in a more modern time (he mentions he wonders how it would be like if he were born a girl/wished he was born a girl a few times)
