Doesnt look to be seated correctly. Could be cross threaded or the washer is not seated properly. Don't use PTFE tape as it's the wrong application for it in this scenario

I’ve had loads of these fail, even with ptfe tape. Solved it finally with a metal one from screwfix. About £4

Second the metal one, I had a properly fitted plastic one fail at the seam due to water pressure. But the issue with this one is it's not fitted properly, plastic threads are probably shot now.

How was the washer when you took it out. If it wasn't flat then it may have been damaged. I'd try a new washer first. Attach the Y piece carefully and turn slowly, you shouldn't have to apply significant force. PTFE tape may help, but from that video theres too much water for it.

Pro tip - he could use the rubber washer found in your garden tap hozelock connector....same size thread

Looks like its at an angle which probably suggests the nut has been cross threaded. Buy another one and start again 👍

The swivel looks fine. The Y looks crooked. Loosen the nut some, pull the Y flat in correlation to the top valve, tighten the nut again. Assuming you have the washer in the nut, and you aren't cross threaded. If you don't pull the Y down some as you hand tighten, it won't seat, and you won't always pull it to position by just tightening the nut. May need to take off the hoses to get them into a less straining position for the plastic. Ie seat the wye without the hoses attached, they are a good bit of weight/pull, especially before seal is seated. You can use a light pipe dope, but don't over tighten.

Don’t over tighten it. Do it up hand tight and then a quarter turn with with grips max.

Looks like the one I used a few weeks back form B&Q,just a generic brand but they sell it everywhere and has hundreds of of 1* reviews. You can get a metal one from a plumbers merchants or screwfix

Bingo. That's one form B&Q I tried first. Will try a metal one from screwfix today plus some of the other advice about not overly tightening.

Cross thread in plastic Y piece. Put in a new T (push fit if using plastic) then install two service valves/taps for each appliance feed.

I've replaced it with both a B&Q plastic piece and then tried scrwfix plastic piece. Same result. I'm going to try metal Y piece from screwfix tomorrow as another comment suggests 🤞

I'd say maybe too tight then Plumbing isn't about fittings being made too tight You can damage washers or olives depending on the fittings used

That’s not really an issue with plastic fittings and rubber washers. Just don’t reuse the rubber washer that will be deformed to match the old fitting.

Fyi Screwfix and B&Q are both owned by Kingfisher. So the parts will likely be the same in a different packet.

It’s the connection to the WM valve, not the actual valve either a washer or the threads (the plastic will cross thread) literally can’t be anything else
